For 403b's the simplest way is to have your accounting and/or HR person contact TIAA-CREF; they are the standard-bearer for 403b plans. They have helped thousands of nonprofits and academic organizations set up 403bs.
For 401Ks I would agree with the other poster's recommendations for Vanguard and would add Fidelity, but not for 403bs.
